We have included a link here to our New Patient Packet for your convenience. Please take a moment to print, read and complete these forms before your first visit to FMCG. Don’t worry we understand these forms can be a little overwhelming at first glance. Included you will find several “consent” forms asking for your consent or permission to access records from other providers, insurance companies, and other agencies which may be needed to provid the most comprehensive care possible for you and your family. You will also find release forms allowing FMCG to provide some of these organizations with information in regards to your care at FMCG.

- If you or your family at any time feel as though your privacy has been violated by anyone at FMCG (Providers, Staff, another Patient, etc.) we want to know immediately.
You must have these forms completed prior to being seen for your initial appointment at FMCG. Your provider will use a portion of the package during your initial assessment, our billing staff will use a portion in completing all the required insurance filing and our scheduling department will use some of these forms to set up your initial as well as subsequent appointments.
