  • FMCG strictly adheres to all HIPPA, Federal, State Federal, State, and local laws in regards to our patient’s privacy.
  • If you or your family at any time feel as though your privacy has been violated by anyone at FMCG (Providers, Staff, another Patient, etc.) we want to know immediately.

You must have these forms completed prior to being seen for your initial appointment at FMCG. Your provider will use a portion of the package during your initial assessment, our billing staff will use a portion in completing all the required insurance filing and our scheduling department will use some of these forms to set up your initial as well as subsequent appointments.

FMCG will assist you in every way possible navigating your policy and understanding your insurance coverage/benefits for mental healthcare.

Patients are responsible for providing FMCG with their most current and up-to-date insurance information.

Patients are responsible to report changes in carries, coverage, co-pays, etc. prior to their next scheduled appointment at FMCG.
